About Formasty

Formasty is a form builder aimed squarely at lead qualification rather than general-purpose data collection, which shows up in what it does after the submit button. You build branded multi-step forms in a visual editor with 20 field types and 19 starter templates, previewing the live form as you edit, and share them as a hosted link or in one of four embed modes — inline, popup, slider or fullscreen. The qualification layer is the actual product: conditional paths branch on earlier answers, weighted scoring assigns points per answer and groups respondents into tiers, variables carry state across steps, and each outcome routes somewhere different — a qualified lead lands on a booking page with the calendar open, everyone else lands on a nurture ending with resources and an email follow-up. The same weighted-rules engine drives product recommendation quizzes, where each result gets its own screen with its own copy and image. Responses flow out through webhooks, Slack and Google Sheets, with submission analytics and exports. The AI angle is that forms can be built and edited by an agent: it exposes an MCP server and an API so Claude, ChatGPT, Codex or Cursor can create and modify forms directly, which is a genuinely different authoring path from clicking through a builder.
